Students participating in interscholastic athletic programs, including intramural sports, shall have an annual physical examination prior to tryouts and participating in any scheduled practice or contest. Physicals are effective for 395 days from the date of the exam. The physical exam form for District 54 can be found by clicking on the link. For additional information on District 54 Junior High Sports and the Sports Parent-Student Handbook please click here.
Physical Examinations:
State law and district board policy requires students to have a current physical examination and all required immunizations on file with the school nurse in order for a student to tryout and/or participate in interscholastic sports or intramurals. The exam must have been administered within one year prior to starting that grade level. The exam must be documented on the Certificate of Child Health Examination form and is the only acceptable form for the physical exam. The medical provider must complete the physical exam and sign and date the medical portion of the form. Parents are required to complete the heading (name, address, etc) and complete and sign the Health History section of the form. Incomplete forms will not be accepted. Current physicals are effective for 395 days from the date of the examination. A copy of the physical examination form along with additional information can be found at https://sd54.org/healthservices/
Message from our School Nurse – Excused from Physical Education and/or Interscholastic Sports
Written notes are required for a student to be excused from physical education or outdoor activity. A parent can write a note and is valid for up to three (3) days following an illness or injury. The note must indicate the injury and length of time the student should be excused. Any restriction for more than three (3) days requires written verification from a physician detailing the type of injury, type of restriction and duration. If a student participates in interscholastic sports or intramurals, and has either a parent note or physician note excusing them from participation in physical education, the student will not be able to tryout or participate until the time period for the excuse is completed OR an updated written note is received. In addition, if a student is on crutches, a doctor note is needed.
